Child Lead Poisoning Case Management

Provides services for children between the ages of 1 year and 6 years with elevated blood lead levels. Families are connected with environmental, community, educational, and medical services. Care is coordinated with the child's provider and Medicaid Health Plan as appropriate.

Children's Special Health Care Services

Assists children and young adults (ending on their 26th birthday) with certain eligible medical conditions as determined by CSHCS doctors. Families of all incomes are eligible to join, including those with insurance. Some families may pay a fee to join; Payments are adjusted using a sliding fee schedule based on household income and family size.

Enrollment begins during pregnancy and continues with mom, baby, and family through the baby's first three years of life. We are here to provide support and resources when needed.

Safe Sleep Matters Program

Sexually Transmitted Disease Control & Clinical Services

Provides confidential testing and treatment for syphilis, gonorrhea, and chlamydia. Clinical staff contact county residents with untreated infections to assist in obtaining treatment. Free Rapid HIV testing with same-day results is also available. Free condoms are available upon request.

Tuberculosis Control

Provides TB patients with care coordination to ensure the patient is fully treated and contacts are tested and evaluated.

Women, Infants, and Children (WIC) Food Program

Provides nutritional counseling, formula, breastfeeding classes, hemoglobin and lead testing, food benefits, health screenings, social service referrals, community garden, and other services to help moms and their babies maintain good health. Services are provided to women and their children from conception up to age 5. There are eleven WIC offices throughout Out-Wayne County.
