This form is used by the Indigent Defense Services Department (IDSD) to determine attorney caseloads on a rolling calendar basis. Attorneys are required to update this form quarterly. “Updated” information should only be submitted if it differs from your most recent submission. If nothing has changed since your last submission, you do not need to select “Updated” for that section. Please review your records carefully before completing this form. Any changes must be reported promptly to ensure accurate caseload tracking. Caseload information may be shared with other courts or jurisdictions in accordance with IDSD policy.
